End with the place of publication and the name of the publisher.

Take the information about the book from its title page and copyright page. If more than one place of publication is given, use only the first; if more than one date is given, use the most recent one.

Give the city and state (abbreviated) for all US cities.

Give the city and country (not abbreviated) for all non-US cities.

Include the province (not abbreviated) for Canadian cities.

Do not give a state if the publisher's name includes it (as in many university presses, for example).
